Health Forms

Health Forms for Retreat Participants

Youth Health Form

Adult Health Form

A Health Form MUST be brought to the camp for every camper for each program they attend. If medical problems occur, the camps of Crossways use local clinics and hospitals to treat medical conditions or accidents. Medical expenses will be billed to you through these clinics. Crossways does not carry medical coverage for campers or retreat participants.

Adults: At this time, we do not require health forms for adult participants, but we STRONGLY ENCOURAGE each adult to complete and turn in a health form for their stay at camp.

How is Crossways responding to H1N1 concerns?

We are aware of the threat of H1N1 and Influenza A flu and are asking our guests to take the following precautions to aid in the prevention of this and other contagious illnesses.

Each retreat participant (and present parent) will be asked if they have a fever, a sore throat or a cough. They will also be asked if they’ve been exposed to someone with these symptoms. If anyone answers "yes" to fever AND one or two of the other symptoms we will ask that the child stay at home because of an influenza-like illness.

If anyone in your family has a fever and a sore throat or cough we recommend that your camper does not attend camp. Please call our office to discuss cancellation and look into any alternatives available.

Specifically for Groups

  • If a member of your group is exhibiting flu like symptoms, please do not allow them to come to camp as they are probably contagious at least 24 hours prior to onset of full symptoms appearing according to the CDC.

  • Prior to your group's arrival at camp, please instruct your group in healthy hygiene techniques as proper hand washing is essential in preventing the spread of illnesses. Wash hands often and especially before meals.

  • If necessary, teach your group participants healthy cough and sneezing techniques. Please try to sneeze or cough into your arm or sleeve, instead of your hands. Flu is spread through droplets.

  • If any one in your group is exhibiting any of the following symptoms while at camp, have them see one of our retreat staff as early detection and separation may help in preventing others from becoming ill as well. Some flu like symptoms include: fever, headache, cough, body aches, vomiting and/or diarrhea.

  • Please let us know if someone from your group does become ill while at camp, as we have has infectious illness policies and procedures that are put into effect anytime we have multiple guests or staff with similar symptoms. All cabins, lodges and the dining hall are disinfected after every group departs but please inform our staff should a member of your group become ill or is sent home early so that we may disinfect your cabin once the ill person(s) has left for home.

Our goal is for you all participants to have an amazing experience at Crossways and following these simple guidelines may help you avoid getting sick. If you have any questions, please feel free to contact our Appleton Office at 920-882-0023 or the camp you will be attending.
